Here is a brief history of major Jedi and Sith lightsabers, their appearance and how they lost.
Darth Maul
Darth Maul appears as a confidant of the mysterious Sith Lord in everything, and undertakes Qui-Gon Jinn and Obi Wan Kenobi on multiple occasions. He is the only Jedi or Sith before or after being seen with the deadly double lightsaber, which is only revealed as such during the epic Battle of Naboo. During the fighting, half of the lightsaber double with disabilities, as Obi Wan fight for survival, is finally able to send Darth Maul, with him and his double lightsaber was lost in the depths.
Obi Wan Kenobi
Obi Wan Kenobi leads to multiple lightsaber through the Trilogy of principles as well. As with Darth Maul and his lightsaber, Obi Wan is lost through a deep hole in the final struggle. It is only after the recovery of Qui-Gon lightsaber that is able to kill Darth Maul. Neither the sheet is again.
In Episode III, Obi Wan Kenobi has a new lightsaber that appears throughout the fight with Count Dooku, again against General Grievous and finally against Anakin. This lightsaber itself is gone, not to be seen again until that is Ben Kenobi on Tatooine, where he disarms a bully in the cantina in Mos Eisley, and finally finds his final against Darth Vader in the Death Star. It shows what happens to Kenobi's weapon, but assumed that his former pupil kept or disposed of it, and never seen again.
Anakin Skywalker / Darth Vader
Speaking of Anakin from Episode III, his lightsaber making it the end of the fight against Obi Wan Kenobi. Obi Wan lightsaber saves this, eventually giving Luke Skywalker in Episode IV.
Presumably, when Anakin becomes Darth Vader at the end of Episode III, he gives his all black again, red-leaf lightsaber used in Episodes IV, V and VI of the expedition Kenobi, Luke dismember, and is lost when dueling with Luke again aboard the Death Star, dismembered, in turn by Lucas. Darth Vader lightsaber moves off the shelf that are struggling to not be seen again (and presumably destroyed when it bursts the Death Star).
Luke Skywalker
The lightsaber Luke, by Anakin in Episode III, serves him well In Episode V against the Wampa on Hoth and finally disappeared (like so many in Star Wars), from a high altitude in the City Cloud when it is cut by the hand of Darth Vader.
Luke is a lightsaber again between episodes V and VI (the green instead of blue), used to make their way from the Sarlacc in Episode VI and use in their final battle against Darth Vader and the Emperor. When the Emperor tells Vader finish, Luke throws his lightsaber away on the platform, but keep it off.
Count Dooku
Single hilt lightsaber Count Dooku makes his first appearance in Episode II to assume Obi Wan, Anakin and Yoda and finally again in Episode III against Obi Wan and Anakin, and eventually used against him in his death at the hands of Anakin. Presumably lost during escape from the ship is on board.
Yoda
Yoda goes to demonstrate their mastery of the Force in Episode II against Count Dooku and other Once in front of Episode III Darth Sidious. Its leaf is green, much shorter than the other Jedi weapons, but packs the punch it. Lost in Coruscant during the battle with the Emperor and Yoda never see again in episodes V and VI.
Mace Windu
Lightsaber Mace Windu single color purple is used to great effect in the arena on Geonosis in Episode II and against Darth Sidious in Episode III. He is disarmed by Anakin and Windu's lightsaber away from the streets of Coruscant then followed shortly by Windu himself.
Common themes
Lightsabers are common trends in the six films, including the loss of high places (Darth Maul / Kenobi - Episode 1, Yoda / Windu â € "Episode III, Lucas â €" Episode V Darth Vader â € "Episode VI).
Often, the dismemberment of the hand or arm of arms (sun cho) is also a common method of loss or defeat the Jedi and Sith alike. Anakin loses a hand to Dooku in Episode II, then returns the favor by cutting off Dooku's hands both before to kill him in Episode III. Also Windu Anakin loses a hand, before being sent by the emperor.
The break is the shadow of Anakin in Episode V, when Luke loses a hand to Darth Vader, and then returns the favor in Episode VI by reducing Vader's hand. This is the action that he keys in the downward spiral potential of their father, and he falls short of following his destiny as the Emperor sees.
